Policies

Effective date: October 29, 2026

The policy below is section 2 of the Boulder Biologics Administrative & Financial Agreement, word for word. You sign that agreement before we schedule or provide your services, and the signed agreement is what governs.

Deposits, Cancellations, and Rescheduling

We may ask for a deposit to schedule a procedure. You understand that:

  • The deposit is applied to the cost of your procedure.
  • The deposit is nonrefundable.
  • If you cancel your appointment, you forfeit the deposit.
  • If you reschedule your appointment to a later date, the deposit is applied to the new appointment. This includes rescheduling less than 48 hours before your appointment.
  • A deposit applied to a rescheduled appointment can be used for 6 months from the date of the original appointment.

Late cancellations and missed appointments

  • If you cancel less than 48 hours before your appointment, or you do not come to your appointment and do not tell us, you forfeit the deposit and you may be charged a late cancellation fee.
  • The late cancellation fee is up to 50% of the estimated fee for the procedure on your estimate. Your deposit counts toward this fee.
  • If we are able to give your appointment time to another patient, the late cancellation fee is no more than your deposit.

When we waive the late cancellation fee

We do not charge the late cancellation fee, and we apply your deposit to a new appointment, if:

  • We cancel or reschedule your appointment.
  • Your provider postpones your procedure for a medical reason.
  • You have a medical emergency or are admitted to a hospital.
  • A member of your immediate family dies or becomes seriously ill.
  • Severe weather or road closures make travel unsafe.
  • The Director of Operations approves an exception.

We may ask for documentation of a medical emergency, a hospital stay, an illness, or a death.

To cancel or reschedule, call us at (720) 550-6175.
